GAME CARD RESPONSIBILITY FOR COACHES
In the competitive divisions (10U and older), coaches must provide a Game Card to the referee before the game is permitted to start. The Referee will use the card to track substitutions, score, injuries, cautions and any other information pertinent to the game.
The Game Card must be completed in advance of the game and contain details of the time and location for the match.
It must also contain the names of the players, in order of jersey number. And the name of the coach and of your opponent's coach.
Blank Game Cards are available in the Important Documents section of the
ayso2.org
homepage.
After filling in the card, both sides must be printed. You must print onto card stock.
Here is an example of what a correctly completed Game Card should look like before you surrender it to the referee prior to kickoff.

At the end of the game, the the coach should
always, always, always
, ensure that the referee plans to surrender the card back to your Division Coordinator using a previously defined process. This process should have been communicated to you by your Division Coordinator. If you are unsure, please contact your Coordinator for clarification. The process will vary from division to division, coordinator to coordinator and field to field.
